Consent Form

​I, the undersigned, understand and agree to the following terms and conditions:

•​I am at least [18] years of age and have provided accurate information about my age and personal health.

•​I acknowledge that getting a tattoo involves risks, including but not limited to pain, discomfort, bleeding, infection, scarring, allergic reactions, and other complications. I understand that the tattoo process may cause physical and emotional reactions.

•​I take full responsibility for informing the tattoo artist of any medical conditions, allergies, medications, or complications that may affect the tattoo process or healing.

•​I acknowledge that the tattoo artist has explained the aftercare instructions, and I agree to follow them diligently to ensure proper healing of the tattoo.

•​I understand that the tattoo artist cannot guarantee the final result, as the healing process may vary from person to person.

•​I consent to the tattoo design, placement, and size as described in this form.

•​I agree to release and hold harmless the tattoo artist, their employees, and the tattoo studio from any liability, claims, damages, or expenses arising from or related to the tattoo process, healing, or any complications thereof.

•​I authorize the tattoo artist and the studio to take photographs or videos of the tattoo for promotional purposes, without any compensation to me.

 

•​I certify that I am not under the influence of drugs or alcohol at the time of signing this release form.

•​I have had the opportunity to ask questions and have received satisfactory answers regarding the tattoo process, risks, and aftercare.
